Duplicate Custom Entries

For some reason every time I generate a custom entry (i.e. monster, spell, etc.) it creates a duplicate. For example I created the custom entry “Skill” and whenver I select it again it generates a second custom type of skill, which means the drop down menu gets full very fast and needlessly full.
