Table of contents based on headings

Where a document includes headings (h1, h2, h3), it’d be nice to have a table of contents that shows the structure of the document. Clicking on a particular heading in the table of contents (TOC) would scroll to the correct spot in the document. This would make navigating longer documents much easier.

Maybe the table of contents could be shown/hidden by clicking a button in the toolbar.
