Cannot create multiple blank chapters in advance (might be an nvda issue but unsure)

What happens:

Users can no longer create multiple empty chapters ahead of time. Chapter creation appears tied to cursor position or current chapter state.

Expected behavior:

Users should be able to create blank chapters freely, regardless of cursor position.

Impact:

Limits non-linear workflows such as outlining or pre-structuring manuscripts. Increases friction for planning-heavy writing styles.

This functionality previously existed, suggesting a possible regression or an nvda failure to see the button.
